1) Not enough air flow so you can't breath all night
2) No forehead support
My solution came in 2 steps, as follows:
First Buy the "Memory Face Cradle Pillow Sherpa Covered Face Pillow by Core Products" (sold here on Amazon.com for $46.99)
This is the type of pillow that massage therapists use on their tables. The pillow supports your forehead and cheeks in very soft memory foam. The chin area is open so it's comfortable for extended periods of time.
Second The height of this "Memory Face Cradle Pillow" is 3" so your mouth is very close to the mattress. In order to increase the air flow around the pillow, you need to raise the pillow up off of the bed about 2" by placing the pillow on a platform. So to make a platform, I went to my local home improvement store and bought one 6' long board that was 2" wide and 1" thick. (cost $4.13) I sawed the board into 3 pieces to form a U shape and then sawed four 2" pieces for the legs of my platform. I nailed these pieces together and the end result was a little wood platform to sit my "Memory Face Cradle" pillow on top of. Now, because the pillow is resting on top of this platform and not directly on the mattress, there is airflow all around the pillow. Without this platform, the foam pillow will squash down as you sleep so your mouth will be right on the mattress. The only way to ensure air flow is to build the wood platform and sit the pillow on top of this platform.

The pillow is generally as advertised. It should be strongly advised to the consumer that this pillow cannot be used well on a bed without making some provision for breathing. A spacer of some sort is needed if this is not used on a massage table type device with a hole for breathing. The pillow cover does have velcro sewn into it, but it included no velcro to mate it with, so the pillow opening can widen and leave your face falling through. I am so glad I got this for Christmas! I suffer from chronic neck pain due to a series of car accidents, so my husband often gives me massages at night to help ease the pain. I hated trying to position pillows "just right" so I could breath while facing down because it never worked and I couldn't relax all the way because I was so concerned with breathing. Now that I have this, I can breath while faced down, relax, and enjoy my massage. (Oh, it's so wonderful!) The reason I didn't give this product five stars is I do have to agree with a previous reviewer that the memory foam collapses so your face almost touches the ground (so it gets hard to breath after a while), but what I do is roll a towel to put under it so my face is still raised even if the foam squishes down. Bottom line: if you are a person who wants to breath while getting back or neck massages while faced down, this pillow is for you! First things first. This is not really meant for use as a pillow for anything other than a massage table. Secondly, this pillow is wonderful. It cradles your face like a cloud and allows for total relaxation of the neck muscles. It's very soft and holds its shape quite well. It's very tempting to use it without an additional cover as the sherpa covering is so soft and supple. It has 3 velcro strips on the bottom of the pillow to attach to most (if not all) massage table cradles.The table I purchased had a very bad face cradle pillow that left your face numb after only a few minutes. This pillow has turned the only negative about my table into an absolute selling point. Highly recommended!
