I am 6'1", 170# guy, with sleep apnea. My entire life I have done everything I can to crane my neck and head while I sleep so I can breath. I also have used CPAP for a few years (Thank goodness for CPAP!!!), but I still need to crane my neck and head back.
I have used the 3" Diameter roll for about a year now, and don't travel without it. I stick in in my pillowcase under the edge of my pillow, it it works well, but of course it tends to move around at night, and has gone about 50% flat over the course of a year, which are my only two complaints. Like I said, I can't live without it, and it's much better than any other product I have tried.
I recently purchased the 5" diameter roll. It is too big and bulky and difficult to position, but I am thinking it will eventually compress 50%, and then be just about right, right???
All in all, this is a great product at a great price, and I would highly recommend it. I suggest you start with the 3" roll, unless you are very tiny, or very large!If you reading this, chances are you are dealing with cervical issues, discomfort and/or pain. These are reasonably priced and worthy of your consideration.
I was in a rollover car accident in 2006; fortunate to walk away largely uninjured other than a quarter-sized burn on my left elbow. I did have a fair amount of soft-tissue trauma in my neck from the roof caving in on my head. As a result of the rollover and sports injuries, I have arthritis in my neck which has caused constant discomfort for several years, and recently, it got to the point where the discomfort was becoming intolerable. I purchased a heating pad which fit over my shoulders and around my neck, would do stretching exercises throughout the day, several times a day, and while those helped somewhat, it is this pillow the 5 inch one that has made the most significant difference. It sure is an improvement over using icy hot and patches on my neck they have their place in terms of lasting relief.
I place it across the bottom of my pillow at night, under my neck. The side of my head rests on the pillow, and this foam roll ensures my neck is supported and level. Now, instead of waking with an incredibly stiff neck and discomfort, I wake with very little stiffness and no discomfort. The difference for me is profound. I rest better, and I wake feeling better.
The foam is not overly rigid and yet not overly soft. I think it fits the bill "just right".
There are a couple of different sizes; I am 6'3" and on the muscular side. I would guess a petite or smaller-statured person might use the smallest, and someone in between the middle size.

I bought this to replace one that was worn out but it is too hard. It is very uncomforatble and cover is flimsey.This item was not advertized by size. If I had known it was as big as it is, I would not have bought it.This pillow is far too large to use as a cervical pillow. I can't imagine that it would ever compress enough to allow my neck to be in a neutral position. I use it for other things. It's kind of handy, just not for the neck.
