I like this pillow best for the car. Once it's in place, having the external bulb makes it very easy to inflate/deflate even while driving. A downside is that it can catch on things if you're constantly moving it between the car and house/office.
The $19 version is pretty much the same pillow, however the bulb and release valve are just sewn into the pillow underneath "+" and "-" symbols. Being self-contained, it's definitely easier to move around and take with you. However, it can be hard to reach the bulb to inflate, and you can also accidentally lean against the air release valve when you don't intend to.
Overall, I'm happy with both pillows, but this version is probably slightly better. It'll give you a little better support in my opinion than the $19 version. I keep this one in the car, and carry around the self-contained pillow for the house and office.My car did not come with built in lumbar support. So I ordered this item, opened the back of my driver's seat, inserted this lumbar support and left the tube and bulb hanging out toward's the right side of my driver seat. It works great! I now have adjustable lumbar support for my aching back. Nice product! You will need to consult an owner's manual to figure out how to get into your seat back. And please be careful...as modern cars like mine have wires in the seat for air bags, seat belts and seat warmers and you don't want to damage or pinch those.

Does the trick for so many airline seats that have "negative lumbar support" Sometimes I wonder if the designers of some airline seats have actually sat in them for more than 5 minutes...This product seems really well made and easliy fits in my backpack...

I am very satisfied with the quality of this lumbar pillow. It looks and feels very well made. I was a bit concerned the pump action would be gimmicky and not work well, but I am very pleased with how it works. You feel a clear expansion of the pillow with each pump of the bulb. The packaging doesn't include any real instructions, but I'd say 7-10 pumps would be the absolute max (I didn't want to risk maxing mine out I'd think up to 5 pumps would be what most people would use). This is nice that you just need to pump it a few times to get the expansion you need before receiving it I pictured myself having to furiously pump the bulb. Also, even fully deflated, the foam inside provides some support that might be all some people need.The only things different than the picture is that the cushion I received is black. Also there is an adjustable strap on it to fasten it to your chair-back, which is not pictured, but I assumed it would have.
The company selling this item Colonial Medical Assisted Devices shipped the pillow super fast. I received it within 2-3 days of placing the order.
The car I drive has about zero lumbar support in the seats so I am hoping this cushion will work under an aftermarket seat cover. I haven't started trying it out yet, but I previously bought a larger memory foam cushion, but it was way too big pushing me forward too much. I am hopeful this one will work out with its smaller size. I would surmise that your end-result level of success with this cushion will depend on your individual need and usage of it. I would reiterate that it is a very well-made product for the money and is sold by a good company.This is a good product and performs as advetised. I would have liked it to be a little wider across my lower back. Also note: it provides considerable support even when the bladder is totally empty of air. If you want to remove all support, you have to take it out. Otherwise very comfortable, also makes a nice neck pillow when you want to tilt you seat back and take a nap.
